This style of transplantation is so unique, Dr. Bolton cannot even charge by the graft because it simply doesn’t translate. Instead, he charges by hair volume. What you describe is called "Telogen Effluvium". Telogen effluvium is when a stress (such as dieting/weight change) causes noticeable shedding of hair after the stressful event. The hair loss is always delayed, usually 3 to 4 months after the "stressful event".
The event can be a physical stress such as an illness (especially with a fever), sudden weight changes (the most common), or a major surgical procedure like gastric bypass surgery. Sudden severe emotional stress can also trigger telogen effluvium, including the death of a loved one, a divorce, or a job termination.
It's all about something called "Transition to Resting Phase". In telogen effluvium, the stressful event causes a high proportion of hair follicles to halt their growth phase prematurely, and to begin a transition to the resting phase at the same time. 3 to 4 months after the stressful event, a high proportion of follicles shed their hairs all at about the same time, as they prepare for a new cycle of growth.
The condition is almost always temporary, and new hairs soon grow back in. Within a few more months the normal random cycle of hair follicle growth and rest resumes. Treatment for telogen effluvium includes waiting for the new hairs to grow in, and styling to give a fuller look in the meantime. Make sure your diet is TRULY TOTALLY sugar free. Sugar greatly exacerbates the condition. It also helps to do the following:
* Up your protein by 15% each day over whatever levels you've been eating.
* Make SURE you are taking in at least 1500 calories per day.

I think the idea is to get hair transplants as the hair is receding, and thinning, not to wait till all the hair has turned into fine nearly invisible vellus hairs. There is a lot of information on different doctor's websites regarding this. One thing that is helpful to know is there are currently at least two popular methods to harvest the hair to transplant. Strip extraction surgery aka follicular unit strip extraction surgery(FUSE) involves cutting a strip of hair bearing scalp from around the back and sides of the head. I thin it is around 10" long by about 1/4" wide. The scalp is then stapled or stitched together, obviously there is healing and a scar and some discomfort. The density remains the same in the harvested area; the transplanted area can never reach the same level of density. The strip of scalp is dissected into 1, 2 and 3 hair follicular unit grafts under a microscope, that are then transplanted. The tightness of the scalp will be a limiting factor to the number of strip extractions. The other method is follicular unit extraction(FUE) aka the woods technique. This involves no scalpels, no cutting and no stitches or staples. The 1, 2 and 3 hair follicular unit grafts are taken from the donor area which is clipped very closely. This thins the donor area so an over all even density can be approached between the donor area and the transplant area. This is a minimally invasive technique and only a topical anesthetic is used. The transplantation of the grafts is the same as with strip extraction surgery and is performed with a syringe like device. I think FUE is more costly, but an FUE guy can probably wear a crew cut if he so desires, since he does not have multiple scars circling the back/sides of the head. Anyway, just search on the internet. There are is a lot of information including videos of the procedures. Another consideration is design of a hairline that will look appropriate as one matures, as well as design of a hairline that might even improve upon the appearance of the face and facial features over the natural hairline. Ask questions and really investigate before going to the expense.
