However if the surface is completely smooth, it indicates that the hair follicle is no longer alive and there is zero chance to grow hair- except in some cases as explained below (Alopecia Areata, Totalis and Universalis).

Non Treatable conditions
The conditions of hair loss which have no chance of hair regrowth are: Alopecia from birth, Scarring Alopecia, Traction Alopecia and Lupus disease.
Alopecia from birth is a rare condition, where the infant is born without hair follicles.
In Scarring Alopecia the infected zone looks like a scar and the hair follicles are dead. The reasons of the phenomena are unknown. The chances of growing new hair are zero, but with proper treatment the expansion of the Scarring Alopecia can be stopped.
In Traction Alopecia the hair is pulled out of the scalp and the hair follicle becomes weak and dies.
In alopecia which is caused by Lupus disease, the skin is rough and pink and the chances of growth of hair is almost non existent.
The conditions of hair loss which has a chance to be treated successfully are: Male Pattern Alopecia, Alopecia Areata, Alopecia Totalis and Alopecia Universalis.
Male Pattern Alopecia is common male baldness. It’s characteristics are loss of hair at the front and crown area first. In many cases, there is still at least hair plume, hair loss and baldness can be stopped by affecting the hair follicles.
In Alopecia Areata condition, typical round hair loss patches evolve rapidly. Sometimes the bald patches spread to full hair loss of the scalp (Alopecia Totalis) or even whole body areas (Alopecia Universalis). The phenomenon could harm men, women and small children. Conventional medicine assumes the Alopecia Areata condition is an autoimmune disease, though the exact mechanism of Alopecia Areata is not known nor fully understood.
In many cases Alopecia Areata is caused by psychological pressure: people who experienced trauma have sometimes lost entire hair within hours or days.
Evaluating Chances of Successful Treatment
As explained in the introduction, the condition of the hair follicles is the most important criteria for evaluating the chances of successful treatment. Existence of thin white hair is a visible sign for living hair follicles. The thin white hair can be noticed when examining the scalp from close distance, with good light conditions. Sometimes viewing the scalp from an angle against light source helps to see the thin white hair.
Naturally, when there is thin white hair on the alopecia area, it indicates that the hair follicles are weak but alive, so there is a basis to grow hair, given the proper treatment. But when the alopecia area is smooth, the evaluation depends on the alopecia type:
In male pattern alopecia condition, smooth scalp is a clear indication that the hair follicles are not alive and there is no chance to grow new hair. However in Alopecia Areata, Alopecia Totalis and Alopecia Universalis the hair follicles sometimes remain alive even if the alopecia area is smooth and evaluating the condition of the hair follicles should take into consideration other important factors.
The factors which play important role in the condition of the hair follicles and hence the chances of success:
– Age of the patient: at younger age there are higher chances to recover
– Period with Alopeia Areata condition: if the condition is recent, the chances are higher. Among people who experienced Alopecia Areata for decades, the hair follicles are probably too weak.
– Alopecia Areata condition was due to a shock: In those cases, the chances of recovery are better.
– Use of violent and inappropriate treatment: Some treatments cause permanent damage to the hair follicles. In such cases, even if there is growth of hair, it is common that once the treatment is stopped the hair falls and the hair follicles remain weaker, which makes it harder to stimulate growth.
– Thin white hair: as previously explained, if there is thin white hair the chances of recovery increase.
– Use of wig: When a wig is applied, the scalp’s skin is not normally exposed to air and is not breathing (skin is not able to absorb oxygen). In cases of wig users, the chances of recovery are lower. If wig is attached by glue, chances are near zero.
It should be emphasized that these are general guidelines for treatment success chances, based on experience with a population of Alopecia Areata patients. A personal evaluation is required in order to accurately evaluate the success chances of a specific person.

Yes, hair restoration is real, and it does work. As far as transplants go, there are two common methods that are used:
1) strip method. They remove a small section of the scalp, usually somewhere between the earlobes, and a few inches above the neckline, on the back of the head. This area is then stitched back together. It may sound a bit gruesome, but it's an outpatient procedure, no worse than going to the dentists office. They anesthetise you, but you are conscious the entire time. Once this strip is removed, technicians divide the strip into groups of folicles, ranging from single to triple. The physician then makes tiny incisions on the scalp, where these folicles will be transplanted to. His assistant(s) will usually start counting, "one, two, three.....two hundred twenty, twenty-one." Each count is an individual incision. The incisions are done in groups, and done quite rapidly. Then, the physicians and assistants implant the folicles into these sites. It's usually an all-day process. The patient is usually seated in a chair like a dentist chair, and they have movies playing, they offer snacks and beverages. The patient may chat with the staff while they work. It's a very relaxed atmosphere.
2) FUE method. The main difference here is that instead of removing a strip of hair, a punch tool is used to extract follicles. This leaves little tiny holes in the donor area, as apposed to a scar area. The sites and implantation are about the same as the strip method. The FUE method is said to be more expensive. The main advantage is that it's less intrusive than the strip method.
In my particular case, they did the strip method, because it was considerably less-expensive, and also because, according to the physician, it provided more donor hair than the FUE method would have.
After the procedure, a couple of days later, the transplants scab over. My scalp looked like I had a very bad case of acne! This is normal. My scalp itched, and the donor area (where the stitches were) hurt a little, but they provided medication to combat this, so the discomfort was minimal. After about 14 days, I had the stitches removed, and life was pretty much back to normal. I could have gone right back to work the day after the procedure, but I decided to take the week off, and just relax. That's what I recommend.
As for the cost, most transplant places charge per graft. A graft is an incision or transplant site where a folicle (grouped as one, up to 4 individual strands of hair) are implanted. My procedure involved approximately 2200 grafts. Depending on where you go, these grafts can range between $5 to $10 per graft. Some places offer a scale, where, the more grafts you go with, the less they charge per graft. Anyway, yes, it's expensive. It's like buying a car. Most places offer payment programs.
Did it work? YES! It took about 5 months before I really started to see the results. The transplanted hair came in very thin at first, and then slowly started to get thicker. No, it didn't give me back that full, lush head of hair I once had. Here's the thing; A lot of people have more than one procedure done, because, depending on the amount of hairloss, and the results they desire, sometimes one procedure isn't enough. I'm very happy with my results so far. Yes, there is still an area on my crown that's quite thin. It looks much better than before though. I will probably have an additional procedure done in a year or two to address that area, and to add some overall thickness.

Research and my experience show that hair loss is inherited from the mothers side of the family.
Regardless, if you have a full head of hair, then the hair loss in front could be caused by reasons other than heredity.
Wearing a tight cap or headband, using tons of hairspray on your bangs and combing through it without washing the spray off first could cause breakage. Using a hair dryer close to your head could cause the hair to burn and frizz up causing miniaturizing. If this hair is burned then you are also burning your scalp so pl keep the dryer no less than 6 inches away from your scalp.
If you use a curling or straightening iron, you could be doing the same thing-using it too close to the scalp. If you damage or burn your scalp nothing you do to restore your hair will make it grow through a burned scalp.
Here are some tips that could help:
Always wash your hair spray off before combing through it.
Never battle with your hair tangles. It causes breakage. Hair stretches about 2 1/2 times its length when it is wet and when you stretch it to remove the tangles, that becomes the weakest point which at first frizzes then breaks off causing split ends.
Do not use aerosol hair sprays, They contain wax and wax builds up and causes breakage. Use a pump spray if you have to.
